Supply your name, company name, shipping address,
billing address, phone and
fax numbers, face length (fl) and
outside
diameter (od) of roll(s) to be covered, quantity of covers
for each roll size, color, price, and Purchase Order Number,
2.
Or order through fax transmission supplying the above information,
3.
Or use the e-mail address
supplying information from section 1a.
If ordering
by fax or e-mail, a confirmation call from Andy's will follow. |
